Step 1 - Complete a Patient Home Delivery Account Form to register.
This can be completed online below or the PDF can be printed here Patient Home Delivery Form
Then e-mailed to info@cortexhealth.com.au.
Step 2 - Ensure your prescription is attached to the registration form. (Original scripts must be posted to us at 85 Argus Street, Cheltenham.
Step 3 - Wait for a Cortex Staff member to be in contact regarding your first delivery order.
